Skilled Nursing Facility

Stable patients that are improving and do not qualify for a rehabilitation program may benefit from a skilled nursing facility admission. Patients must meet defined criteria to qualify for skilled nursing care. The skilled nursing facility can deliver continued medical, physical, occupational, or speech therapies. The move to a skilled nursing facility should be viewed as another step forward or improvement in the patient's condition. The skilled nursing facility may be part of the medical center or a separate facility.

The level of care delivered at the skilled nursing facility should be comparable to the hospital but the facility does not have on-site doctors, laboratory, X-ray, or surgery facilities. The skilled nursing facility team focuses on the last phase of healing and attempts to return the patient back to independent activities of daily living. The patient's condition and progress dictate the duration of the skilled nursing facility stay.

When considering a skilled nursing facility several aspects should be investigated prior to moving your loved one:

 

red dot  The location. This is important regarding the accessibility for visiting.

red dot  Who will be providing the care to your loved one?

red dot  What are the visiting hours?

red dot  Ask for the institution's most recent inspection results.

red dot  What is the availability of beds?
 
red dot  How does the doctors' coverage work?

red dot  Word of mouth references can be very important. Speak to your family and friends. Also ask visitors of current patients what they think of the facility.

red dot  The staffing. Use common sense. Is the staff friendly to you and the patients? Is there high staff turnover and if so, why? Does the staff respond to the patient call lights in a reasonable fashion?

red dot  How does the patient room selection occur?

red dot  Visit the skilled nursing facility prior to moving your loved one. Take the tour and if possible, visit once after the tour to observe routine activities.

red dot  Are there any special needs involved in the care of your loved one - religious, language, dialysis, etc.?

red dot  What is the food like?
